Patients are reminded to provide their self-blood glucose monitoring (SBGM) levels at their regularly scheduled times which are collected in their cloud-based portal. LIFEMAP™ adjusts Care Messaging accordingly based on patterns in their blood glucose levels and participation in the program.
Analyze and adjust
Care providers have direct access to program participation levels and can read and analyze patient SBGM data to further customize the LIFEMAP program and provide more detailed recommendations for diabetes care management.
